Acknowledgements

The Archive has benefited from the work of many different people who have been instrumental in acquiring and processing the patient data entered into the Tinnitus Data Registry (see Data Acquisition staff and Data Processing staff listed below).

The successive editions of the Archive have benefited from the expertise of two individuals who designed and produced the corresponding web sites. Mara Charnell designed and composed the first edition, the Oregon Tinnitus Data Archive, in 1995 (the data content of which is incorporated with only minor changes in this second edition, under the heading Data Set 1). Thomas A. Creedon provided the web expertise for this second edition of the Archive.

The editorial tasks of selecting and formatting the Archive content, adding explanatory and background information (glossary, methods, appendix, etc.) and in addition designing a consistent organizational structure for the second edition of the Archive, have been performed by Mary Meikle with substantial assistance from Thomas A. Creedon.

Jack A. Vernon, Ph.D. (Professor Emeritus of Otolaryngology, Oregon Health & Science University; Founder and past Director, Oregon Hearing Research Center; and Founder of the OHSU Tinnitus Clinic)---Developed the tinnitus testing procedures used in the Clinic; provided the original impetus for establishing the Tinnitus Data Registry; and continued to provide research leadership and support until his retirement in 1996. Special recognition is due for his expert consultation and advice, which contributed in many important ways to the Registry. Without his input, both the scope and the quality of the Registry data would be greatly diminished.

Data Acquisition Staff

Robert M. Johnson, Ph.D. (late Professor Emeritus of Otolaryngology, Oregon Health & Science University (OHSU); and past Director, OHSU Tinnitus Clinic)---Directed all audiological testing and tinnitus evaluation for the Oregon Tinnitus Clinic (1978-1998).

Barbara Lentz, M.S., CCC-A (former Tinnitus Clinic Audiologist, Oregon Hearing Research Center)---Performed audiological evaluation and tinnitus testing, and provided technical consultation for data acquisition (1984-1987).

Sharon Robinson Martin, M.S., CCC-A (former Tinnitus Clinic Audiologist, Oregon Hearing Research Center)---Performed audiological evaluation and tinnitus testing, and provided technical consultation for development of patient questionnaires and interviewing techniques (1981-1982).

Lynn Mahendroo, M.S., CCC-A (former Tinnitus Clinic Audiologist, Oregon Hearing Research Center)---Performed audiological evaluation and tinnitus testing (1983-1984).

Kathryn Storter, M.S., CCC-A (former Tinnitus Clinic Audiologist, Oregon Hearing Research Center)---Performed audiological evaluation and tinnitus testing (1987-1991).

Data Processing and Statistical Staff

William Coshow, M.S. (former Data Analyst and System Administrator, Biostatistical Laboratory, Oregon Health & Science University)---Provided technical support and advice for computer data processing (1981-1986).

Susan E. Griest, M.P.H. (Senior Research Associate, Dept. of Otolaryngology, Oregon Health & Science University; Manager, Clinical Data Processing, Oregon Hearing Research Center)---Has provided oversight and management for all data processing and statistical analyses for the Tinnitus Data Registry (1985-present).

Kuo Hwa Lu, Ph.D. (late Professor of Biostatistics and Director, Biostatistical Laboratory, Oregon Health & Science University)---Provided statistical advice and design consultation (1981-1984).

Linda E. Press, B.A. (former Senior Research Assistant, Oregon Hearing Research Center)---Performed data acquisition from patient charts, data management and statistical analyses (1986-1997).

Nancy R. Schuff, B.A.(former Research Assistant, Oregon Hearing Research Center)---Assisted in the early development of patient questionnaires and interviewing techniques, and performed data acquisition and data processing (1981-1984).

Barbara J. Stewart, Ph.D. (Professor of Family Nursing, School of Nursing, Oregon Health & Science University)---Served as principal statistical consultant for the Tinnitus Data Registry; provided expert advice on methodology and measurement constructs; and made essential contributions to clarity, comprehensiveness, and logical consistency of the statistical analyses undertaken by the Registry staff. (1983-1995)

Lewis Van Winkle, M.S. ( former Programmer and Statistical Analyst, Biostatistical Laboratory, Oregon Health & Science University)---Performed data base programming (1981-1983).

Support Provided by Funding Agencies

Funds to support the development of the Archive have been provided primarily in the form of research support for the staff and facilities of the Tinnitus Data Registry, through grants to the Oregon Hearing Research Center, Dept. of Otolaryngology, Oregon Health & Science University.

Special mention should be made of the role of the American Tinnitus Association in providing a "seed money" grant in 1980 - 81 to support the initial planning and start-up costs for the Registry. The ATA support was crucial in that it enabled the research team to seek and acquire long-term funding for the Registry.
